The .pdf form is a "Fill-In the Blanks Form.  Place the cursor on the area in which you wish to type, enter your information. 
To select a Check Box, place your cursor over the box, the hand will change to a hand with a finger pointing to the box, left-click the mouse or press the space bar and your check mark is there.

If using the "Free" Adobe Reader, you will not be able to save your form. 
Print two copies:
One for your records and one to turn in.

If Save Is Selected:
Open the "saved file"
After completing the form, click save again.

If Open is Selected:
Complete the form.
Print a copy to turn in and a copy for your records.
